Book an appointment
Book an appointment
Tell us what is going on and when suits you. The practice comes back to confirm a time that works — you will not be left wondering whether it went through.
Online scheduling
Choose a time in the practice’s scheduling system. The appointment is confirmed by that system, and any change or cancellation goes through the same place.
Rather talk to someone?
For anything urgent, call. A form is never the fastest route to being seen.

Book an appointmentPrefer to talk it through first? Calling is often quicker, and you will speak to the practice.
Before you come in
Wear something you can move in
The assessment involves moving, bending and turning. Ordinary comfortable clothing is fine — nothing special is required.
Think about what you want back
The most useful thing you can bring is a clear answer to “what would you be doing if this were not an issue?” It shapes the whole conversation.
Arrive a few minutes early
Enough time to complete intake without rushing. If intake has been sent to you in advance, completing it beforehand gives that time back to the appointment.
What to bring
- A list of any medications you take
- Relevant imaging or reports, if you already have them
- Your insurance details, if you plan to use them
- Contact details for other providers involved in your care

How long to allow
Allow time for a first appointment to be longer than a follow-up. The practice will tell you how long when you book — the site does not state a duration, because that is a scheduling fact about one practice rather than something a template can know.
Where your health history goes
Health history and insurance details go through the practice’s secure intake, not through a public form on this website. Nothing on this site asks for a medical history, and you should not send one by email.
